Description
Helps veterans achieve success and avoid homelessness following release from incarceration in state or federal prison. Provides outreach and assessment services; referral services to connect veterans to clinical, mental health, and social services, such as job assistance; and short-term case management support.
What This Program Offers
HCRV services offered to veterans prior to release from incarceration in state or federal prisons, including outreach and assessments to assist with planning for community re-entry. The Veterans Justice Outreach Program (VJO) conducts outreach to veterans involved with the earlier stages of the criminal justice system.
HCRV services provided after release from incarceration include:
- Referring and connecting veterans to healthcare, mental health, and social services, like job assistance
- Short-term assistance with case management
