Purpose

Provides funding to evaluate, develop, and expand the use of technology-enabled collaborative learning and capacity building models to increase access to healthcare services in rural, frontier, and tribal areas and other underserved communities. Promotes distance health education models to connect healthcare providers with multiple other healthcare professionals and specialists to facilitate case-based learning, disseminate best practices, and evaluate outcomes.

Required activities include:

  • Evaluate, develop, and as appropriate, expand the use of a technology-enabled collaborative learning and capacity building model to improve retention of healthcare providers and increase access to healthcare services in a rural area, frontier area, Health Professional Shortage Area (HPSA), or Medically Underserved Area or Medically Underserved Population (MUA/MUP), or Native Americans
  • Develop instructional programming, and train healthcare providers and other professionals that provide or assist in the provision of services through technology-enabled activities, such as
    • Telementoring models such as Project ECHO, ECHO-like models
    • Distance learning
    • Clinical decision support
    • Other emerging models
  • Provide support through such models to healthcare providers and other professionals that provide or assist in the provision of services
  • Perform information collection and evaluation activities to study the impact of the models on patient outcomes and healthcare providers, and to identify best practices to expand and use the models

Applicants must address at least 2 of the following focus areas:

  • Chronic diseases and conditions, including Alzheimer's disease
  • Infectious diseases
  • Mental health
  • Substance use disorders (SUDs)
  • Prenatal and maternal health
  • Pediatric care
  • Pain management
  • Palliative care
  • Other specialty care

Amount of Funding

Award ceiling: $475,000 per year
Project period: 5 years
Estimated number of awards: 8
Estimated total program funding: $3,800,000

Who Can Apply

Eligible applicants include public or private, nonprofit or for-profit entities, including faith-based and community-based organizations, tribes, and tribal organizations located in U.S. states and territories.

Eligible organizations must provide, or support the provision of, healthcare services in rural areas, frontier areas, Health Professional Shortage Areas (HPSA), or Medically Underserved Area or Medically Underserved Population (MUA/MUP), or Native Americans, including Indian tribes, tribal organizations, and Urban Indian Organizations.
